A fifth person died Sunday from injuries sustained in the crash of a bus and a tourist "duck boat" in Seattle last week, a Harborview Medical Center spokeswoman said.

The 20-year-old woman, who wasn't identified, was an international student attending North Seattle College, the spokeswoman, Susan Gregg, said in a release.

Four other students from the same college were also killed. Earlier Sunday, hospital officials said that five people were still in intensive care at Harborview; four were in critical condition and one was listed as serious.

The death was reported less than an hour after National Transportation Safety Board member Earl Weener said the 1945 military-style duck boat, which had been refurbished in 2005, hadn't received a recommended axle repair.

The company that fixed up the vehicle a decade ago, Ride the Ducks International, notified customers of the "safety related concern" in 2013, Weener said, adding: "We haven’t had the opportunity to determine if the Seattle franchise had received the information."

In a briefing Saturday, Weener described an axle as having "sheared off — for what reason yet we don't know."

The accident occurred on Seattle's Aurora Bridge just after 11 a.m. Thursday. The duck boat and a charter bus had been moving in opposite directions on the bridge when the boat appeared to "lurch suddenly," a witness told NBC station KING on Thursday.

"I saw a bunch of smoke and what appeared to be the front left wheel pop off, and then it clipped a smaller SUV and basically almost T-boned into the oncoming bus and spun around," said the witness, Jesse Christenson.
